Witness statements

A witness statement is a written statement signed by a person which contains the evidence which that person would be allowed to give orally.
Start by saying: I, Mr ....., will say as follows:

Then start numbered paragraphs.

In paragraph 1 say why you're making a witness statement.
In paragraph 2 say "In summary I invite the court to make the following order..." and set out the basic order you want.

For the rest of the statement explain why you want the order you want.
Remember the judge is busy and needs to get to the point fast so they can deal with other cases. Less is more.
